The first form is for a single, affected dog, the litter sheet is for multiple dogs affected in a litter OR for a breeder to provide information on a litter (normal and abnormal). 

 Always provide DNA blood samples from affected and unaffected  littermates, parents, grandparents whenever possible! Talk with your breeders to get them to help in the study! The more DNA sent in from a family tree, the more is able to be compared. If sending in DNA from a unaffected dog, please simply write "UNAFFECTED" in big letters across the submission form
